How are these estimates calculated?

Transfer gas × current gas price × ETH/USD = estimated USD fee

Transfer gas: the median gas used by up to five successful, direct transfers of each token, found in the latest three blocks. A small sample may not represent your transfer. If none are available, the page labels a generic 65,000-gas fallback.
Gas price: the next-block base fee plus the median of the 50th-percentile priority tips across the last ten blocks. This is an estimate of the effective price, not a maximum fee cap or a confirmation-time guarantee.
Dollar conversion: Chainlink’s most recent ETH/USD price. It updates periodically, not on every trade. Prices over two hours old are omitted.

The recipient’s token balance and the transfer’s execution can change gas usage. These estimates cover a simple transfer, excluding swaps, bridges, approvals, exchange withdrawal charges, and smart-wallet overhead. Fees are not calculated as a percentage of the amount sent.
